BOOKING
- A deposit is required at the time of booking to confirm your appointment (will go towards your total).
- This deposit is refundable or transferable within 48 hours of your scheduled appointment
- It is NOT transferable to another person
PAYMENT/PRICE
- Cash, e-transfer and all major credit cards are accepted.
- Please note, a 3% processing fee will be added to your total for all card payments.
- No refunds.
RE-SCHEDULING/CANCELLATIONS
- Clients must provide at least 48 hours notice via text to (647) 407 - 5036 to cancel or reschedule the appointment.
LATE/NO SHOW
- If you are going to be late or cannot make it to your appointment due to unforeseen circumstances, please contact as soon as possible via phone call or text to (647) 407 - 5036
- Clients who arrive over 15 minutes late (with notice) may result in a deposit forfeit and cancellation of your appointment.
- Clients who arrive over 15 minutes late OR do not show up to the appointment without giving any notice will be considered a ‘No Show’ and will be required to pay 100% of the booked service before scheduling another appointment.
EXISTING WORK
- If you have existing permanent makeup, please send photos to info@permanentbeautybysam for a consultation before booking an appointment.
RESULTS/GUARANTEE
- Individual results vary from client to client and are extremely dependent on how you heal.
- Although premium products are used and high quality service is provided, there is NO GUARANTEE for permanent makeup results.
- Factors such as client’s aftercare, skin type, and personal lifestyle will directly contribute to the final results.
- Some may require additional touch ups (charged accordingly) to achieve their desired results.
PLEASE NOTE: PERMANENT MAKEUP (COSMETIC TATTOO) SERVICES ARE NOT RECOMMENDED FOR CLIENTS WHO ARE OR HAVE ANY OF THE FOLLOWING:
- Pregnant or nursing
- Under 18 years of age
- Diabetic
- Glaucoma
- Skin diseases such as shingles, psoriasis, or eczema on the area of treatment
- Viral infections or auto-immune diseases
- History of keloid/hypertrophic scarring
- Skin cancer
- Hemophilia
- Had an organ transplant
- Epilepsy
- Use of any skin medications such as Accutane in the past 6 month
- Allergy to epinephrine, dermacaine, benzyl alcohol or lidocaine
- Undergoing or undergone Chemotherapy/Radiation in the past 6 months
